Cornerstone Family Healthcare is actively recruiting for a Patient Navigator and Support Specialist to join our growing team in Newburgh, NY.

RATE OF PAY/SALARY: $23.58 per hour

WORK LOCATION(S): Newburgh, NY

STATUS: Full Time


  • Manages care coordination related to cervical cancer screenings, colonoscopies, FIT tests and other related preventative screenings.
  • Reaches out to patients who have missed preventative screening appointments.
  • Collaborates with all of the patient’s providers, within the Cornerstone Family Healthcare system and externally, to ensure all pertinent patient information, including information on cervical and/or colorectal cancer screening and other preventative screenings, is included in patient medical records and accessible via EHR.
  • Will complete written assessments that identify and address potential barriers to patient completion of cancer screening.
  • Will monitor and track patient progress in completing Cancer screenings, Diagnostic testing within 60-90 of positive screening, Initiation of cancer treatment, as applicable
  • Report findings on a regular and timely basis
  • Establishes and maintains community resources as needed
  • Educates patients on the importance of preventative screenings and positive health outcomes.
  • Assist in translation for Cornerstone Family Healthcare staff when necessary.
  • Participate in mandatory Patient Navigator trainings
  • Implement Evidence Based Practices with the goal of increasing colorectal and cervical cancer screenings by the end of the project date.
  • Be familiar with HIPAA laws and Cornerstone Family Healthcare confidentiality policies and ensure that they are followed by Medical Records employees.
  • Attend and participate in department, All Staff and other required meetings.
  • Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations.
  • Perform other related duties as assigned.
Requirements:
  • High School Diploma
  • Bilingual (English and Spanish)
  • Knowledge of EMR
  • Some medical experience preferred
