Position Overview:

The primary function of the Population Health Specialist is to work as part of the patient care team in providing high quality, efficient and patient centered services while demonstrating the health center’s core values. Working alongside physicians, mid-level providers, registered nurses and medical assistants, the health specialist assists with the coordination of the case for health center patients. Employing population management techniques, the Population Health Specialist is responsible for coordinating health care and enabling services activities.

*Proof of vaccination for COVID-19 or approved exemption must be presented prior to beginning work with Sandhills Medical Foundation, Inc. Full vaccination is receipt of both doses of the Moderna or Pfizer vaccine or one dose of the Johnson and Johnson vaccine.

Responsibilities:

• Coordinate activities necessary to successfully meet the requirements established by managed care plans
• Manage caseload to maximize patient utilization
• Educate patients on rights and responsibilities
• Identify patients that require more intensive case management
• Educate patients on community resources
• Meet FQHC performance measures

Education: Bachelor of Social Work, Health Education, Licensed Practical Nurse, Medical Assistant

Location: Several positions are available in Camden, Lugoff, Bethune, Kershaw, McBee, Ruby, and Sumter area

Pay: $17.71 - $20.27 per hour
