Job Summary:

The Clinical Coordinator is responsible for staffing clinical home health cases timely, accurately, and with great consideration to schedule the appropriate staff member to the patient/client. The Clinical Coordinator acts as a liaison between staff and family and vice-versa. The Clinical Coordinator ensures that all visit ordered frequencies are followed per the patient plan of care and over communicates with Clinical Management and clinical field staff to ensure that care delivery is met exceeding expectations of the patient/client and ensure Help at Home follows Agency policy and procedure and is in compliance with accrediting body, Medicare, Medicaid, local, state, and federal guidelines, and regulations. The Clinical Coordinator may be crossed trained to schedule for the non-skilled home care client population.

Duties/Responsibilities:

Schedule Coordination
  • Coordinates client and employee schedules by understanding care plans, ordered frequency, and schedules patients/clients with the appropriate clinical team member(s)
  • Positively communicates with patients/clients and family, encouraging them to ask questions and helping them explore ideas and ways to improve their Help at Home experience creating Great Starts, Meaning Moments, and Great Days!
  • Communicates with patients/clients regarding satisfaction with employees and service
  • Nurtures and maintains positive relationships with patients/clients
  • Identifies opportunities to improve services to patients/clients, new and existing, using positive customer service techniques
  • Reports situations involving complaints, incidents, or risk and injuries to employees immediately to the Clinical Manager
  • Communicates regularly with direct care employees, noting good work performance, identifying problems, and communicating issues to the Clinical Manager
  • Develops strong working relationships with patient/clients, field staff, internal ancillary office staff, case managers, and referral sources
  • Creates and modifies system schedules accurately and timely, always maintaining up-to-date staff schedules
  • Runs reports from the system to enhance effectiveness of care delivery as required
  • Participates in staff meetings and may provide reports in preparation for team conferences or as otherwise directed by the Clinical Manager/Director of Clinical Services
  • Monitoring completion of visits and completed documentation and notifying Clinical Manager regarding any untoward trends identified
  • May be on-call after hours for assigned Agency or service area as needed and assigned
  • Light clerical and office duties as needed
  • Performs other related duties as assigned


Required Skills/Abilities:
  • Excellent organizational skills: ability to multitask and manage multiple responsibilities
  • Strong problem-solving skills
  • Excellent verbal communication skills, primarily on the telephone
  • Mature demeanor needed for directing others on cases
  • Better than average ability to use standard Microsoft Office software and scheduling software
  • Proficient with Microsoft Office Suite, especially Excel


Education and Experience:
  • High school diploma or GED
  • LPN preferred
  • Minimum of one (1) year of experience working in the health field or medical office environment
  • Previous experience in schedule coordination, preferred
  • Previous experience with working with the elderly and/or disabled or must display a compassionate attitude for this population
  • Valid driver's license


Physical Requirements:
  • Prolonged periods sitting at a desk and working on a computer
  • Ability to participate in physical activity with the potential for moderate lifting, bending, standing and/or sitting on a regular basis
  • Ability to travel between assigned locations as needed
