Summary:
The Population Health and VBP Program Manager role supports the organization’s commitment to value-based and patient-centered health care. They are responsible for overseeing patient navigators and coordinating the workflow in the department working with payors and vendors to meet established quality measures for all value-based payment programs.
Duties and Responsibilities include the following. Other duties may be assigned.
- Responsible for all Value Based Payment opportunities and the potential financial benefits of meeting all requirements for each plan, on an annual basis.*
- Provides a summary of all VBP plans from a clinical perspective and requirements.*
- Reviews workload versus payment opportunity and analyzes with CEO and practice.*
- Responsible for the build in Medent (Practice Electronic Medical Record System), if required, and operationalizing registries in Medent to meet the needs of the practice.*
- Responsible for development of implementation program to include training of nursing staff and operationalizing quality metrics.*
- Responsible for identifying resources needed to implement selected VBP programs.*
- Provides monthly reporting to CEO regarding the current status of VBP programs being pursued in the practice.*
- Ensures AWV is completed for all eligible patients.*
- Develops practice workflows to insure success in the selected programs.*
- Participates in maintaining PCMH Level 3 certification for the practice, annually.*
- Works to achieve maximum success in Excellus ACQA quality metrics.*
- Captures 90% of all available reimbursement for VBP programs selected by the partners.*
