Under supervision, the class works primarily in the field, providing professional nursing care to individuals, groups and families to improve the overall health of communities, or functions as a nursing team leader in a health clinic; and performs related duties as required
Essential Duties for Family Connects (FCC)
Using telehealth and home visiting, the nurse will deliver the Family Connects service model to families who have welcomed a newborn into their home. The comprehensive Family Connects visit consists of medical, psycho-social, and environmental assessments centered on the birthing person and newborn. Once the assessments are complete the nurse will connect the family to resources based on the families wants/needs, using a resource finder. All visits are documented in Salesforce. The nurse will also provide education on health education, disease prevention, nutrition, child care and related health topics. Nurses will participate in weekly multi-disciplinary case conference to share experience and clinical and resource opportunities. The nurse will also work with community health providers, social service agencies and other healthcare providers to plan, implement and establish standards for client care.
Essential Duties for Tuberculosis
Under the supervision of a PHN III, the PHN II works as part of an interdisciplinary team to provide case management for individuals on treatment for active TB disease, which includes performing in-home visits, providing education on tuberculosis and its treatment, checking medication regimens for appropriateness and monitoring for adverse effects, tracking lab results, identifying barriers to treatment completion, linking patients to pertinent community resources, communicating with treating clinicians, and participating in monthly case conferences. Tuberculosis nurse case managers also provide directly observed therapy, place and read TB skin tests, and assist with large on-site screening events (e.g., at a school or worksite).
Essential Duties for Youth Settings
Work with a multidisciplinary team, including contact tracers, medical directors, and epidemiologists to assist in outbreak response and prevent school and childcare-associated infection; participate in field visits to school and childcare settings to: 1) review practices and assess risk to student safety, 2) collect clinical case information and review medical records, 3) assist with specimen collection in the field (e.g., nares, skin, stool samples), 4) develop training materials and conduct training for staff at schools and early childcare centers. 5) Collaborate with other CDPH Programs to connect Schools and Early Childcare Settings with other resources available within the Health Department. (Youth Settings Program PHN II)
Lead Poisoning and Prevention
PHN II will provide case management and home visit services to a case load of children 6 months to 16 years and pregnant people with elevated blood lead levels. During home visits, the nurse will conduct developmental and social emotional assessments, perform a physical and nutrition assessment, assess the community needs of the family, provide education on nutrition, hygiene, the effects of lead poisoning and lead hazard reduction techniques and provide information on the importance of lead testing, monitor medication treatment plan, collaborate with environmental inspectors to identify the source of lead poisoning, and document all information collected in the Healthy Homes and Lead Surveillance System. Nurses will also perform workshops trainings at daycares, health fairs and other community groups. The nurse will collaborate with social workers, WIC, primary care providers and Illinois Department of Child and Family Services to assure that children with blood lead levels above the reference level receive appropriate services.

THIS POSITION IS IN THE CAREER SERVICE
MINIMUM QUALIFICATIONS
Graduation from an accredited college or university with a bachelor's degree in nursing accredited by the National League for Nursing (NLN) or an approved foreign credential evaluation, plus one year of professional nursing experience.
Licensure, Certification, or Other Qualifications:
Must be licensed to practice as a Registered Nurse. Applicants must possess a valid State of Illinois Registered Nurse license at the time of employment.
SELECTION REQUIREMENTS
This position requires applicants to complete an interview which will include a written exercise and/or a skills assessment test as part of the interview. The interviewed candidate(s) possessing the qualifications best suited to fulfill the responsibilities of the position, based on the oral and written parts of the interview will be selected.
Preference will be given to candidates possessing the following:
- Previous work experience in a healthcare setting for under-served populations.
- Intermediate Proficiency with Excel.
- Previous work experience in a clinical role within an acute care setting.
- Previous experience working in the field.
- Previous experience with maternal and/or child health and home visiting.
- Familiarity with HIV and sexually transmitted infections
- Bilingual in English and Spanish or Cantonese or Mandarin.
