The Dietician is responsible for development and implementation of program, objectives, planning and implementation of the SPHC diabetes self-management education (DSME) program; evaluation of the DSME program outcomes to direct continuous quality improvement efforts; provides case management services for a sub-population of patients with diabetes, e.g. newly-diagnosed, patients who are pregnant, diabetes in youth, uncontrolled T2DM, etc.

• Provides direct outpatient with diabetes
• Diabetes self-management training with group and individuals
• Glucose monitoring education
• Provides medical nutrition therapy
• Provides MNT and DSME services as needed

NECESSARY QUALIFICATIONS:

• Bachelor's in Dietetics
• Registered Dietician with CDR

• Commission on Dietetic Registration
• Certified Diabetes Care & Education Specialist or BC-ADM
• BLS from American Heart Association
