Black River Healthcare Clinic has an immediate opening for a full-time Chronic Care Management and Visiting Registered Nurse position. The CCM/Visiting RN position plans and provides comprehensive patient care and care planning both in the home and outside of the home. Manages available nursing resources and collaborates with other members of the healthcare team to provide quality cost-effective patient care. This position is:

• Full-time, 72 hours per pay period, day shift Monday thru Friday, and on call

Essential Duties:

• Conducts the initial, comprehensive assessment and has knowledge of eligibility requirements for admission to a home health program and chronic care management program.
• Provides appropriate direct care, in the patient’s location of residence, to the patient through the respective program (i.e. wound care, IV site care).
• Maintains availability to provide telephone advice per protocol, and handles urgent and emergency calls during working hours and during non-working on-call time as scheduled.
• Demonstrates knowledge, expertise, and skills to competently provide care management services. Maintains at all levels of case management, a sensitivity to cultural diversity, identified barriers, patient’s rights and ethical treatment, psycho-social needs, clinical condition, safety and security of physical environments, and treatment plans.
• Identifies services required for chronic care management patients that cannot be provided within the program scope. Locates and collaborates with alternative providers and facilities as required.

Educational Qualification:

· Bachelor of Science or Associates degree in Nursing

· Minimum of 1-year nursing experience

· Experience with Hospice and/or Homecare preferred

· Must have a valid WI driver's license, proof of insurability, and reliable transportation

· BLS within 6 months of employment

· Current Wisconsin RN License
