A reputable healthcare system in New Hampshire is searching for a Registered Nurse for its Family Medicine department. In this role, you will triage patients, assess symptoms, plan interventions, implement treatment plans, evaluate responses to interventions, and revise care plans based on overall effectiveness. Responsibilities:

• Provide direct and indirect patient care.
• Document symptoms and vital signs, responses to interventions, and report progress to providers.
• Triage phone calls from patients, analyzing the urgency of needs.
• Document the care provided and the patient’s progress toward the desired outcomes.
• Educate patients and families on the care plan and how to manage care after discharge.
• Administer medications to patients as needed/directed.
• Collaborate with interdisciplinary teams to improve patient care processes and outcomes.

Qualifications:

• An ASN is required, and a BSN is preferred.
• Active RN license.
• 2+ years of clinical experience.

• Schedule; Monday-Friday, 8a-5p
• Pay; $35-45/hr., depending on experience + shift differentials.
