We are seeking a full-time
Home Health RN to join our healthcare team in
Lynwood, WA. The Home Health RN will provide skilled nursing care to patients in their homes, including assessments, treatments, and patient education. The RN will work collaboratively with the healthcare team to ensure the patient's needs are met and documented accurately.
Responsibilities:
- Admit clients to home health services by completing start of care documents, including but not limited to initial nursing assessment, home safety assessment, fall risk assessment, and plan of care.
- Prepare and submit clinical notes at the end of each visit. We do not take Medicare/caid clients, so the documentation is so easy.
- Provide ongoing home health visits, RN delegation, and RN care management.
- On-Call on a rotating basis. Tasks include doing phone triage, emergency visits, and opening.
- Conducts nursing skilled orientation, verifies per diem nurse’s competency of skills, and conducts supervisory visits of nurses as well as annual performances.
Requirements:
- Active RN license in the state of Washington.
- Minimum of 1 year of experience in the home health field.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively with the healthcare team.
- Valid driver's license and reliable transportation.
- Basic computer skills and familiarity with electronic medical records.
