CHCW has an opening for an RN Care Coordinator to join our CONNECT team. The CONNECT Program offers medication-assisted treatment (MAT) for patients with opioid dependence and opioid use disorder. The Care Coordinator works with patients to plan & monitor care for medication-assisted treatment (MAT) and facilitates patient access to appropriate medical and specialty providers.
Job duties include:
- Assists care providers during MAT clinics and provide patient education.
- Schedules patient appointments and coordinates care related to MAT services.
- Manages the waiting list and assists patients waiting or entering the program.
- Provides clear and concise documentation of each element of planned care goals.
- Provides teaching to patients/families related to substance use disorder (SUD) treatment with Suboxone.
Qualifications:
- Current WA State RN License required.
- Two (2) years of related nursing experience are required.
- Prior experience with Substance Use Disorder is preferred.
- A current driver’s license and auto insurance are required.
