We're hiring multiple full-time Registered Nurse Case Manager for our growing Hospice team in Bellingham, WA.

JOB SUMMARY:

The RN Case Manager is responsible for the planning, coordinating, providing, and documenting care for a caseload of terminally ill patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Registered Nurse Case Manager develops and implements a hospice plan that meets each patient’s specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines, as well as the Agency’s policies and procedures. Participates in Quality Assurance Performance Improvement (QAPI) activities.

QUALIFICATIONS:

• Graduate of an accredited nursing program.
• Current Registered Nurse licensure in the state of employment.
• Experience as an RN in Home Health, Hospice and/or Acute, preferred
• Current BCLS certification
• Current license from the state Board of Nursing.
• A valid driver’s license, reliable auto, and current auto insurance.
