Salary: $55,000-$75,000 plus Bonuses.
Hours: Variable depending on Community events, outreach and admissions
Duties:
- Increasing referrals and admissions within assigned territory
- Establish relationships with local hospitals, skilled nursing facilities, clinics, and other referral sources including caregivers, Nurse Practitioners, Home Health Aides, LVNs and RNs
- Proactively seeking out potential clients
- Manage specified client case with ability to solve concerns and issues.
- Provide appropriate resources to Clients.
- Performs interviews of new team members as needed.
- Admission of new clients
Qualifications:
- Social Work or nursing degree preferred
- 2+ years of experience in long-term care, hospice, home care, palliative care, or hospital environment required.
- Prior experience as a customer service representative in healthcare
- Excellent skills in communication, organization, and relationship building.
- Aptitude for sales
- Proficient in Microsoft Office
- Willing to travel to North San Diego County for meetings and other team events
