Provides direction, supervision, and evaluation of social services for referred patients of all age groups (pediatrics, adolescents, adults, and geriatrics) and from all areas of the organization (Inpatient and Outpatient).

Job Duties:

• Integrating social services/ case management functions into patient care throughout the entire continuum of care, which encompasses inpatient and outpatient areas.
• Mobilizes resources to assist in achieving desired clinical outcomes within the desired timeframe
• Identifies, coordinates, and refers for specialized services, education, and support programming as assessment warrants with community resources and agencies.
• Communicates with physicians regularly and develops an effective working relationship.
• Act as a patient advocate and provides social services that ensure a patient and family achieve the maximum benefit of healthcare services.
• Provide supportive counseling and referral services to patients and staff.
• A key member of the hospital’s Health Equity Program development, implementation, and evaluation.
• Leader of our patient and family advocacy group.

• Master’s degree in social work from an accredited social work program
• Experience in clinical practice including individual, family, and group psychotherapy
• Clinical Social Worker (member)
• Current BLS certification
• Physical/emotional Stamina
• Communication/writing skills
• Thorough knowledge of Joint Commission, state and federal regulations relating to Case Management and discharge planning
• Interpersonal skills

Work Schedule: Monday - Friday; 8-hour shift
