Responsibilities / Accountabilities

1. Initial evaluations and screens are completed and documented within 24 hours of admission for subacute residents and 48 hours for long-term care patients.

2. Quarterly or significant change screens are performed within established time frames as assigned by gathering information from staff, resident, family, medical records and functional observation. Residents requiring therapeutic intervention are identified and referrals to other services are made.

3. Residents with significant risk for aspiration are identified and appropriate recommendations/referrals are made and communicated with all team members immediately for safe swallowing.

4. Appropriate means of communication with residents are established and effectively communicated to all team members.

6. Consistently maintains timely and accurate departmental and medical records according to department guidelines.

7. Discharge planning is provided and documented for residents, caregivers and/or staff as indicated by the discharge plan. Training can include specific assistance techniques for all areas the resident requires assistance with, use of all adaptive equipment prescribed and home exercise programs.

8. Relevant continuing education or in-service are provided to facility staff as assigned.

9. Participates in CMI activities as requested by the Rehabilitation Director.

10. Openly communicates and facilitates an atmosphere of caring, respect and team spirit throughout the department and facility.

11. Observes facility dress code and maintains a professional appearance.

13. Comply with patient confidentiality and Federal Resident Rights.

14.Adhere to productivity requirements, complete productivity reports and submit in a timely manner.

16.Maintain a positive work atmosphere by demonstrating and communicating in a professional manner so as to foster positive relations with customers, clients, co-workers and managers

15.Perform all other duties assigned by the department supervisor, lead therapist or regional

Job Skills -

1. Current licensure in Speech-Language Pathology in the State of New York

2. Must be able to work during established hours of operations.

3. Must demonstrate knowledge of current best practice therapy procedures.

4. Must be able to move, lift, walk, stand, twist and bend as required by a normal caseload without medical limitations.

5. Must be able to communicate clearly (verbally and in writing) in the English language including communication with sensory and cognitively impaired residents, effective communication within the department and facility and with referral sources and outside agencies.
