Job Title: Suburban Metro Area Continuum of Care - Housing Advocate (Anoka & Washington Counties)
Department: Housing Program
Reports To: Housing Program Manager
FLSA Status: Non-Exempt
Summary: The Housing Advocate is responsible for development, implementation, and coordination of services for individuals who have experienced sexual exploitation and/or victimization who are referred to, or are residents of, the Breaking Free Permanent Supportive Housing Programs. This position is specific to the SMAC region - with emphasis in Anoka and Washington counties.
Duties and Responsibilities include the following. Other duties may be assigned.
1. Completes client intakes, needs assessments, goal planning, and progression of services needed and received for clients who have experienced commercial sexual exploitation and other debilitating dependencies (For example, substance abuse). Meeting with clients within their homes is a requirement of this position. *
2. Connects clients with quality supports based on their individualized needs and as outlined in the client goal plan. This could include but is not limited to services such as mental health supports, chemical health resources, applying for public assistance, safety planning, and transportation. *
3. Maintains confidentiality of all client information in accordance with Breaking Free's confidentiality policy.*
4. Provides appropriate independent living assistance for women and their families, assisting women to normalize routines and schedules of regular daily activities.*
5. Networks with other agencies to provide assistance for women as needed.*
6. Completes service logs and all necessary paperwork to document hours of service, training, etc. and submit to designated staff.*
7. Maintains client files in compliance with state and federal guidelines.*
8. Assists with public awareness activities and other special events.*
9. Provides occasional transportation of clients to and from Breaking Free sponsored events, groups, and meetings, medical appointments, or other services related to goal planning.*
10. Collaborates with property owners and property managers of Breaking Free's master leases to assess and ensure client's needs are being met in their current housing situation, including timely maintenance request follow up, safe living conditions, and adherence to the policies of Breaking Free's Permanent Supportive Housing program.*
11. Participates in applicable internal and external meetings.
12. Any other duties assigned and deemed necessary for the effective and efficient operation of the agency.

Computer Skills:
To perform this job successfully, an individual should have knowledge of Microsoft Office applications including Word, Excel, and Outlook. Individuals should also be comfortable using Internet Software including web-based database applications.

While performing the duties of this job, the employee is regularly required to deal with a high crisis volume environment, talk or hear. The employee is frequently required to stand, walk, sit, use hands, reach with hands and arms. The employee is occasionally required to deal with possible triggers with lived substance abuse experience, climb or balance, stoop, kneel, crouch or crawl, taste or smell. The employee must occasionally lift and/or move up to 20 pounds.
The noise level in the work environment is usually moderate. While performing the duties of this job, the employee will occasionally encounter crisis situations that may create a louder workplace.
