Support Coordination is a very rewarding field, but is also very demanding. Support Coordinators must have a strong work ethic and personal integrity. This is a home based position with field work, so excellent time management skills are a must. The hours are flexible, but this is a full time position and requires some evening and weekend work. All documentation is recorded in the web based system so each coordinator MUST have a computer WITH internet access. Ensuring the health and safety of the individuals is primary responsibility of the job. Face to face contact on a regular basis is required, so reliable transportation IS essential.
Specific Responsibilities:
- Must work 40 hours per week which may include some evenings and weekends (must have flexibility to work from your own home and set your own schedule)
- Manages a 40 person caseload
- Collaborates with families, physicians, and community providers on the behalf of the individuals served
- Contact individuals monthly completes documentation within 4 days / 96 hours of visit
- Facilitates meetings for individual support plans and documentation
- Assist individuals with developing community supports and obtaining Medicaid
- Attends monthly web based staff meetings and quarterly face to face staff meetings
- Twenty hours per year of annual training required
- Travel required, therefore MUST own reliable transportation
- Must independently manage time to fulfill a productive work day
- Must meet established timelines
Requirements: Bachelor's Degree in a Health, Social Services, Nursing, Education, or Related Field.
Minimum of one year of experience working with persons who have a developmental disability or a related disability.
