Preferred Primary Care Physicians is currently recruiting for a full-time Value Based Care Manager position for PPCP's Central Administrative Office located in Canonsburg, PA. The ideal candidate will have a Bachelor's Degree in healthcare, business, or related field, as well as a minimum of 3-5 years of clinical experience and at least 2 years of experience in Electronic Medical Records training. Preferably, the ideal candidate will have excellent written and verbal communication skills, and management experience is a plus. This position is a salary exempt position. General Overview of the Value Based Care Manager position:

The Value Based Care Manager provides daily support and oversight to PPCP Quality Care Consultants (Coordinators) and Transition of Care Liaisons for operational tasks at the direction of the Director.

Location:

Primary onsite location will be PPCP Central Administration office in Canonsburg, PA but position is flexible and allows for remote work capabilities after candidate is fully trained as decided by Director.

Value Based Care Manager Essential Duties/Responsibilities:

  • Regularly evaluate and supervise performance of QCC and TOC Liaison staff
  • Offer feedback, assistance, and problem resolution to staff concerns or questions
  • When needed, assist and offer support to staff to meet quality initiatives.
  • Provide training to new or established staff in need of additional development
  • Serve as interim QCC or provide floating coverage should a need arise
  • In collaboration with the VBC Director, review, analyze and update all areas of the QCC and TOC Liaison staff workflow, including but not limited to: Staffing levels; Recruitment for openings; Employee performance; Completion of audits; Management of staff based on departmental and organizational goals; Workflow productivity and efficiencies; Enforce policies and procedures; Manage PTO requests and staff coverage needs
  • Serve as main point of contact for staff questions and/or concerns related to job role and evaluate if communication back to manager is necessary
  • Distribute communication to staff as directed by manager
  • Monitor the reporting capabilities of Cedar Gate Central Worklist to promote and participate in the education of the tracking tool to the staff related to their workflow
  • Monitor TEAMs chat for staff questions or concerns related to job role and address as able
  • Attend monthly Quality Committee and/or Management meetings as determined
  • Run and analyze reports related to QCC or TOC Liaison workflow as determined by VBC Director
  • Lead monthly QCC meeting if Director is out of the office or unavailable
  • Represent PPCP when in the community and support organization's goals and values
  • Interact in a manner which is professional, respectful, positive, customer-focused and which promotes trust
  • Maintain professional growth and development.
  • Provide leadership by exhibiting confidence in self and others; inspire and motivates others to perform well.
  • Maintain a working knowledge and adhere to applicable federal/state regulations including but not limited to laws related to patient confidentiality, release of information and HIPPA.
  • Other duties as assigned.

Education/Experience/Skills:

  • Bachelor's Degree in business, healthcare, or other related fields preferred.
  • Minimum 3-5 years clinical experience
  • Minimum of 3 years Value Base Care, Care Management or Quality experience preferred
  • Minimum of 2 years of experience in electronic medical records is required
  • Previous management of staff preferred but not required.
  • Strong written and verbal skills
  • Excellent health assessment and communication skills
  • Independent and creative thinker
  • Ability to problem solve
  • Able to work well with Office staff, Quality staff and providers to problem solve and implement changes
  • Utilize data tracking and trending reports to revise goal setting, strategies, etc.

Credentials:

RN, LPN or Certified Medical Assistant preferred

Special Skills:

  • Familiarity with Microsoft office programs, Email, Faxing, Centricity and Population Health Management Tools such as Enli.
