Looking for a qualified LPN or RN to run our wellness department for our loving community of senior's at our Adult Foster Care Facility, Hampton Manor of Hamburg. We are a level 1-2 care Assisted Living Facility with a max of 70 residents.

Summary:
The Health & Wellness Director is responsible for effective overall management of the nursing
department. By overseeing this department includes ensuring high quality of care is being provided to
residents within the community.
Duties and Responsibilities:
- Responsible for the overall direction, coordination, and evaluation of residents care plans.
- Completes resident care/services based on specific needs of resident.
- Maintains and completes resident care assessments on residents per guidelines of Hampton
Manor and State regulations.
- Continuing education for employees based on resident care needs. I.e., blood pressure
education, oxygen education, insulin education, etc.
- Participate in orientation program of nursing staff to cover objectives of department
needs/requirements. If not available, resident care coordinator will participate.
- Ensures community compliance with medication management procedures and best practice
standards.
- Participates in the interview process of nursing staff working alongside Resident Care
Coordinator(s) to ensure Hampton Manor locations are adequately staffed.
- Conducts pre-move in assessments for potential residents to ensure Hampton Manor can
provide high quality care.
- Maintains cohesive public and professional relationships with residents, resident families,
physicians, vendors, and community members.
- Ensures a safe and sanitary environment for residents, employees, and visitors.
- Investigates all health and wellness department incidences, accidents, and concerns in
accordance too company safety standards.
- When state reportable incidences occur, ensuring that the proper documentation is turned in to
the State within the appropriate timeframe.
- On a day-to-day basis visit resident care stations, resident apartments, and resident common
spaces to ensure compliance with State regulations and Hampton Manor best practices.
- Makes written and oral reports/recommendations to the Executive Director concerning the
Nursing Department.

- Assists with residents with Activities of Daily Living (ADLs), including but not limited to bathing,
dressing, grooming, dining, etc., when necessary.
- Audits resident files and medication carts weekly to maintain organization.
- Perform other duties that may be set by the corporation or specified by the Executive Director.
General Requirements:
- Graduated from an accredited School of Nursing.
- At least 2 years’ experience within Assisted Living Communities.
- Current CPR certification.
- At least 2 years’ experience in a position with management and leadership responsibilities.
- Familiarity with State guidelines relating to Assisted Living communities (HFA & AFC).
- Demonstrates a high level of professionalism and courtesy.
- Demonstrates active listening techniques, seeks feedback, creates, and maintains rapport with
residents, families, vendors, and the community.
