Responsibilities:
Provide social services to residents, including emotional support, counseling, and advocacy
Assist in assessing and identifying the social and emotional needs of residents
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare professionals to ensure comprehensive care for residents
Maintain accurate and timely records of resident interactions and progress
Participate in community outreach and education initiatives to promote the values and mission of ALIYA
