Admissions Coordinator Job at Carlyle Senior Care of Fork
We are looking for an Admissions Coordinator to join our team at Carlyle Senior Care of Fork. Our 111-bed skilled nursing and long term care facility is located conveniently between the Marion, Mullins and Dillon areas of South Carolina. The Admissions Coordinator is responsible for inquiry and referral generation, handling and completing of the admission process, providing internal and external promotional services, and positively representing the facility in the community
Qualifications of the Admissions Coordinator:
- Good organizational and customer service skills
- Prior knowledge of insurance benefits in regards to long term care.
- Desire to work with older adults
Responsibilities of the Admissions Coordinator:
- Maintain communication with hospital discharge personnel, administrators, other nursing homes, physicians, clinics, rehabilitation centers, and home health agencies. This may include visiting residents in the hospital or at home prior to transfer to the facility.
- Collect and respond to all admission inquiries and convert them into a new admission if possible. This may include gathering sufficient preadmission data to determine resident level of care and the ability of the facility to properly care for the resident.
- Provide facility tours to interested parties. This may include assisting families and residents in understanding the policies and procedures of the facility and charges made for services rendered.
- Coordinate all resident admissions by obtaining proper admissions paperwork. This includes completing the paperwork associated with the admission, including any legal documents such as power of attorney or advance directives.
- Work well with medical, nursing and administrative staff to ensure appropriate patient placement. This includes notifying appropriate departments when a resident is admitted and/or discharged.
- Maintain high occupancy rates with a proper mixture of Medicare, Medicaid, private insurance and private pay. This includes confirming that all insurance benefits meet the appropriate standard for admission.
