Job Summary and Qualifications

The Advanced Clinical Pharmacist shall practice the profession of pharmacy with the utmost code of ethics demonstrating the organization’s mission and values. Responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The Clinical Pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing medication therapy within an interdisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner. Adherence to all regulatory and medication safety guidelines are at the forefront of all actions and decisions made within the care process. The Clinical Pharmacist will work directly with the certified pharmacy technician to review home medication lists to provide a streamlined medication reconciliation transition into the hospital. Cost-effective utilization of medications will be reviewed and drafted as part of the inpatient admission medication list and discharge medication list by the pharmacist. As needed, the Pharmacist will provide recommendations and/or literature to prescribers supporting the use of formulary agent, as well as, education to clinicians and patients regarding medication management. Reviews medication orders and quality of technicians work to ensure appropriateness of drugs ordered, dosage, route and interactions. Adjusts medications dosage/schedule as appropriate for patient condition as per policy.

QUALIFICATIONS:

• 
  Graduate of a college of Pharmacy accredited by the American Council on Pharmacy Education.
• BS or Pharm.D degree.
• 3-5 years hospital pharmacy experience with supervisory experience preferred.
• General pharmacy practice residency preferred.
• Current Florida Pharmacist License or License eligible. Board certified.
• Customer service abilities including effective listening skills.
• Critical thinking skills, decisive judgment and the ability to work with minimal supervision in a fast paced environment.
