Education:

  • Current State of Colorado Registered Nurse (RN) License

Experience:

  • One year experience in a health care field or with adults with intellectual and developmental disabilities in a community, private or institutional setting preferred.
  • Two years of management experience.

Physical Demands/Working Conditions:

This position requires frequent walking, sitting and occasional standing and driving; occasional balancing, stooping, bending; frequent handling objects, and reaching with hands and arms; and constant finger movements. The position requires occasional lifting, carrying, pushing or pulling objects up to 25 lbs. The position also requires ordinary talking and hearing capability; full vision capabilities; and the ability to read/comprehend, write, communicate orally, and reason and analyze on a constant basis, and to perform calculations frequently. Work is performed using a computer and standard office machinery daily, and occasional use of a vehicle.

POSITION SUMMARY The Nurse Care Coordinator is responsible for monitoring, assessing, and addressing health and medical needs for Mountain Valley Clients in the Garfield, Pitkin, Eagle, and Lake Counties. The Nurse Care Coordinator supports medical staff in their duties and responsibilities. The Nurse Care Coordinator provides leadership by demonstrating a strong work ethic, solution-oriented thinking, open minded, active listening, good judgement, decisiveness, accountability, and strategic thought.

Training

1. Provide ongoing training to medical and direct support staff on medical and health related matters as needed.

2. Teach, observe and evaluate students on Qualified Medication Administration Personnel (QMAP) classes.

3. Provide ongoing evaluation of staffs competence to perform delegated tasks, medication administration and implementation of healthcare protocols.

4. Participate in activities that contribute to the ongoing development of the professions body of knowledge. Monitoring & Oversight

5. Visit people on site routinely and as needed to assess and determine medical needs and health.

6. Provide medical advice to staff in the event of a medical/medication question, treatment question, medical emergency or development of a serious new health problem to determine treatment outside during regular and after-hours as needed.

7. Attend the IP and/or IDT meetings as needed for persons supported.

8. Complete an annual medical case review.

9. Review vitals monthly for persons supported and notify appropriate staff and/or physicians of any concerns. Health and Medical

10. Exercise informed judgment and use individual competence and qualifications as criteria in seeking consultation, accepting responsibilities, and delegating nursing activity to others.

11. Perform ear lavages, blood draws, flu shots and other nursing tasks as needed.

12. Develop and maintain all individualized healthcare, therapeutic and nutritional protocols in collaboration with health care providers.

13. Coordinate admissions and discharges to/from hospital or long-term care settings, or hospice with MVDS Team Members.

14. Coordinate and facilitate psychiatric services, including follow-up of medical recommendations, order changes, updating informed consents and obtaining dictation.

15. Monitor individuals taking psychotropic medication; conduct tardive dyskinesia screenings annually, monitor efficacy of medications and any possible side effects.

16. If needed, attend or coordinate medical appointments and any follow-up, such as medical treatments and recommendations. Monitor implementation of treatment and recommendations.

17. Obtain written orders from physicians and ensure they meet required standards. Obtain clarification on orders as needed and act as a resource to staff and families in carrying out orders.

Communication & Documentation

18. Serve as a liaison between medical professionals, pharmacies, alternative decisions makers, families, and staff to communicate needs related to health/medical issues of people supported as needed.

19. Regularly attend staff meetings at group homes or day program sites to discuss the healthcare needs of the individuals supported at that site with the direct support staff.

20. Thoroughly document all health/medical related issues/correspondence regarding people supported.

21. Review all written health related communication and follow up as needed to assure timely healthcare for all individuals.

22. Assist medical staff as needed to obtain and maintain complete healthcare records including medical occurrence forms, dictations, test results, labs, orders, and medical histories.

23. Ensure medical communication is respectful, clear, and informed.

24. Immediately report negative interactions or complaints to the Executive Director. Administrative

25. Establish, review, and revise written procedures related to health care to assure compliance with Rules and Regulations from oversight agencies.

26. Coordinate prescription drug disposal in accordance with Department of Health regulations on a regular basis.

27. Administer pre-employment TB tests as needed.
