We are seeking dedicated Care Coordinators to join our team for a Quality Measures Program. This is a fully remote position, allowing candidates from any state within the United States to apply. However, candidates residing in Georgia, Alabama, and Pennsylvania are preferred. As a Care Coordinator, you will play a vital role in improving the lives of patients by ensuring their well-being and adherence to prescribed healthcare measures.

Responsibilities:

• Conduct daily calls to patients enrolled in the program
• Verify medication adherence, daily exercise, and healthy lifestyle practices
• Communicate patient progress to the Care Manager
• Maintain accurate and detailed records of patient

Requirements:

• Excellent communication skills and a caring demeanor
• Familiarity with Concierge Medicine is preferred
• Ability to work independently and efficiently
• Residing in the United States is mandatory

Salary:

The starting salary for this position is $2500/month. Please note that this compensation may be relatively lower in states with higher costs of living, such as New York and California. However, this position offers rewarding and long-term employment prospects.

Workload/Expected Work Hours:

You will be expected to communicate with approximately 50 patients per day. The time required to complete these calls can vary, ranging from two to five hours per day. On average, you will interact with a total of 500 patients every month. The initial month may be more challenging as you establish relationships with patients and familiarize yourself with their specific needs and care plans.

Project Duration:

This is a long-term position, and we anticipate that you will commit to a minimum of 12 months of service with your assigned patient population.

Training:

We provide both in-person and remote training options. If you are located in New York or Georgia, we offer the opportunity for in-person training at our office. Otherwise, all training sessions are conducted via Zoom.
