Description of Position:

Courtyard Gardens Senior Living is 79 unit assisted living facility in Boynton Beach, Florida. We are a family owned business who personalizes their approach to the senior industry. We make a difference in each of our resident's lives by offering a managed care approach to assisted living.

Our care staff is predominately Spanish speaking, so bi-lingual with English and Spanish is preferred.

This is an opportunity for you to learn or grow your experience in the assisted living industry. The selected applicant will grow their skills in both clinical and administration works. The chosen candidate will work hand in hand with the Director of Nursing, who has been working practicing in this field for about 20 years. Our community utilizes Med Tech's for medication passes, which will help grow the Care Manager's leadership skills by ensuring proper medication passes.

This position works as a liaison between residents, resident families, and staff. The nursing supervisor’s duties also include problem solving resident concerns and coordinating care with the Caregivers. The candidate will be part of the management team and will work with the DON to address resident care concerns.

Responsibilities of the Care Manager:

1. Assist with resident schedule creation, such as shower, laundry and break schedules, etc.

2. Assist with conducting family/resident move-in interviews.

3. Assist with conducting potential new resident assessments prior to move-in.

4. Assist with coordinating move-ins with DON and community liaison.

5. Helps ensure 1823's and resident charts are current and accurate.

6. Helps ensure all move-in paperwork is completed in a timely manner.

7. Immediately bring prohibited conditions or at-risk residents in need of a higher level of care to the attention of the DON and Director of Operations.

8. Assists with coordination of care planning with home health agencies on site, working within community policy.

9. Investigates and completes incident reports and documentation.

10. Monitor staff performance, providing or arranging assistance as needed.

11. Read all communication notes regarding the community between the Caregiver shifts.

12. Supervise the medication room and orders, working with and supervising Medication Techs and Caregivers.

13. Ensures nursing assessment of the residents, coordinate additional care, communicate with medical personnel and family members, and follow-up after the emergency.

14. Interact with the resident and family members when a change in the resident’s condition necessitates additional services.

15. Helps ensure proper functioning of medical equipment and adequate medical supplies and completes infection control, safety and pharmacy rounds and audits.

16. Readily available and actively engaged in support of community needs in times of emergency situations and perform duties as assigned.

17. Other duties as assigned.

Job Type: Full-time

Pay: $35,987.22 - $65,000.00 per year
