Care Plan Specialist - RN Hybrid after 6 months Job at Parkway Pharmacy
POSITION SUMMARY:
The Care Plan Specialist is a caring Registered Nurse or Licensed Practical Nurse who is responsible for partnering with the pharmacist to develop a comprehensive care plan to assist the patient to achieve a pre-determined health goal. In addition, the Care Plan Specialist will provide administrative support to the Director of Clinical Operations, including, but not limited to, ensuring Parkway Pharmacy remains compliant with all regulatory agencies governing health care delivery and the rules of accrediting bodies.
The Care plans must be developed cooperatively between the patient and their physician. The Care Plan Specialist must maintain a high level of interaction with Specialty Pharmacy patients and Physicians.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Develops a blueprint to manage specific goals for each specialty pharmacy patient, ensuring goal is measurable and achievable.
- Responsible for the direct communication with patient throughout the plan ensuring patient’s physician is included in the decision-making process.
- All Care plans must include compliance training, patient education and intensive monitoring.
- Revise plans based on follow up, which may include change in patient’s drug therapy.
- Review, acknowledge and document all patient assessments each time drug is dispensed.
- Communicate with patient’s physician before starting to use care plan to ensure agreement.
- Proactive in ensuring patient is financially supported for treatment with their insurance carrier, including managing any insurance changes. Seek patient assistance programs or foundations where patient may be under/uninsured and assists with application.
- Maintains patient confidentiality.
- Performs other duties and responsibilities as assigned.
- Provides clinical support to Patient Care Coordinators to optimize patient interactions
EDUCATION AND EXPERIENCE:
- Graduate of an approved nursing program.
- Active and unrestricted license as a Registered Nurse (RN) in the State of New Jersey.
- One (1)+ years of experience working as an RN in a clinical care setting or a physician’s office.
- Previous medical insurance experience preferred.
- Excellent communication skills both written and verbal along with strong interpersonal skills.
- Must possess a strong sense of urgency and attention to detail.
- Demonstrated ability to prioritize multiple tasks.
- Strong critical thinking and problem-solving skills required.
- Capacity to work with limited supervision and support, in a rapidly changing and fast paced environment.
- Must have demonstrated experience reading medical records, including lab results and other documents.
- Demonstrated ability to document reports and correspondence.
- Demonstrated ability to interact in a collaborative manner with other departments and teams.
- Proficient with Microsoft Office and familiarity with RX30 and Asembia1 (a plus)
