What will I be doing?
- Will formulate the clients Individualized Support Plan to meet client’s needs
- Assists management with ensuring adherence to company policies and procedures as well as state and federal regulations related to program, health care and safety of clients
- Completes and submits requested information and reports within the time frames given
- Ensures appropriate use to allowance
- Communicates regularly with case managers, families and guardians regarding all hospitalizations, incidents, and general well being of the client
- Interacts directly with clients in their caseload bi-monthly by conducting a face to face home visit and documents interaction in the Contact notes
- Maintains open communication with Program Director regarding any unusual occurrences that occurs in the foster home
- Develops and coordinates discharge plan indicating all relevant aftercare needs and plans. Accesses community resources that would be beneficial to client aftercare needs
- Attends regular staff meetings and one on one sessions with direct supervisor to process current status of cases
- Must have knowledge of the Florida Statutes for Child Welfare
- Attends and documents court hearings
- Maintains current and adequate medical record documentation in client’s file. Assures that all required client documentation is in the file, including any foster parent and/or outside agency documentation pertaining to the client
- Maintains a professional appearance and appropriate demeanor when representing Pinnacle Family Services
- Consistently meets deadlines regardless of caseloads; works flexible hours in order to meet clients and organizational needs
- Participates in ongoing training as required by company policy and/or licensure regulations
- Completes other duties as assigned by the Program Director, Licensing Supervisor, and Executive Director
Am I a good fit?
- Ability to acquire through staff training a good working knowledge of procedures pertaining to client’s rights, abuse and neglect
- Must be able to relate well with clients, families and other program staff
- Must have the ability to remain calm in stressful situations
- Must be able and willing to provide supports for children with potentially aggressive behaviors
- Demonstrates knowledge of Microsoft Office products, including use of e-mail, Excel spreadsheets, Word documents and Power Point
- Ability to speak effectively before groups of people
Qualifications:
- Bachelor’s degree in a human services field including but not limited to sociology, special education, psychology and rehabilitation counseling
- One-year experience working with children preferably in child welfare
- Must have a valid Florida Driver’s license. Car registration and vehicle insurance is required
- Must be able to pass a criminal background check at time of hire and maintain this level for duration of employment
