Join Our Team
The purpose of the Case Manager is to assist in providing a safe environment for individuals while they are in the detox unit. Case Manager assists patients adjusting to the Detox program routine. Case Manager documents progress in the medical record and assists the detox patients with all case management actives.

Requirements:
- Bachelors Degree
- Certified Addictions Counselor (preferred)
- Certified Addictions Professional (preferred)
- Minimum 1 year experience with Addiction Treatment
- Minimum 1 year experience with Detox
Responsibilities
- Work with 3rd parties to connect client to appropriate after care setting, which includes:
- Obtaining necessary releases of information (ROI) from clients
- Stay in close communication with clinical team to determine aftercare recommendations
- Contacting potential referring agency
- Coordinate with client, family and receiving provider for successful transition
- Assist with discharge planning and documentation, which includes:
- Follow up with Clinical Supervisor/Clinical Director/Nursing to confirm discharge date and time
- Complete required discharge planning paperwork
- Coordinate transportation for clients transition from the facility to receiving facility if applicable.
- Facilitate interventions with clients displaying behavioral or mental instability and clients at risk for leaving Against Medical Advice (AMA)
- Contacting appropriate referents and family members for which agency has release of information (Informing these parties of clients intent and soliciting their assistance either by phone or in person
- Making contact with Clinical Supervisor/Clinical Director for additional suggestions or recommendations to assist in redirecting client
- Making contact with Clinical Supervisor/Clinical Director to determine if clients are potentially a harm to themselves or others (Baker Act or Marchman Act candidate)
- Monitors each client throughout the daily schedule.
- Enforces program rules and expectations and reports infractions to the Therapist and Clinical Supervisor
- Monitors client conformance with program schedule, reports problems and informs supervisor.
- Makes daily assignment checks and discusses results with each client.
- Monitors client participation and reports problems to supervisor.
- Resolves conflicts through general and supportive advice.
- Provides case management services.
- Maintains the confidentiality of all client and facility activities.
- Complies with Policies and Procedures.
- Accepts and utilizes supervision
- Addresses problems noted by supervisor
- Demonstrates willingness to accept responsibility
- Completes all tasks with concern for work quality
- Performs other assignments as requested by supervisor
Pay: $40,000-55,000 Annually
