The Opportunity

Reporting to the Director of Health and Well Being, the Client Services Manager is responsible for engaging clients of The Partnership, providing comprehensive needs assessments, and implementing a service plan that meets the clients’ needs and is in accordance with The Partnership’s mission, values and strategic objectives around homelessness prevention. In order to provide a high level of care, the Client Services Manager establishes a positive working relationship, engages a network of internal and external partners for referrals while utilizing a trauma-informed and client centered approach. The Client Services Manager is integral to ensuring our clients experience a sense of empowerment and dignity at a time of significant challenge in their lives.


Responsibilities

  • Intake and assess clients seeking financial and emotional support, build rapport and facilitate connections to The Partnership’s programs and services.
  • Conduct comprehensive needs assessments and create mutually agreed upon primary service plans to meet those needs.
  • Provide immediate crisis intervention and safety planning if and when necessary.
  • In collaboration with the Health & Wellness and Housing teams, ensure client’s service plans reflect an holistic approach to their wellbeing and are not singularly focused.
  • Proactively outreach to clients to ensure service plan goals are appropriate and remain relevant to clients’ needs based on consistent conversation and continued relationship building. Re-evaluate clients’ needs as necessary.
  • Assist clients in accessing public benefits and entitlements through virtual support with online applications, onsite support at city-based service providers, and provide advocacy for clients when necessary to ensure they receive the appropriate level of care.
  • Provide information and referrals concerning health, mental health and wellbeing; collaborate to create service plan goals.
  • Present at case conference meetings to ensure a team approach to client care.
  • Maintain agency wide reporting requirements.
  • Participate in external coalitions, networks, conferences and events, and training.
  • Other duties as assigned.

Skills and Qualifications

  • Bachelor’s degree required with 5 years of case management experience utilizing trauma informed and client centered approaches; MSW a plus.
  • Proficiency in Spanish.
  • Direct service experience in an environment providing crisis intervention, homelessness prevention, and/or benefits and income support services, with a focus on client intake, engagement and follow-up.
  • Ability to work effectively and creatively as part of an interdisciplinary team.
  • An understanding of NYC homelessness and housing policy and services, and an appreciation of mental & emotional health and education systems as they relate to housing security.
  • Strong commitment to homelessness prevention and social equity.
  • Demonstrated ability and experience working with diverse teams and stakeholders and commitment to fostering diversity, equity, inclusion and belonging in a multicultural environment.
  • Excellent writing and verbal communication skills.
  • Demonstrated proficiency in MS Office software, Google Suite and CRM systems.
