We are seeking a dedicated case manager to join our collaborative clinical team, guided by the Director of Clinical Programs (LCSW). The case manager will work closely with a group of young people enrolled in our transformative programs. The case manager will become a catalyst for their success and achievements and become a steadfast advocate for their progress.
As a New Earth team member, the Case Manager upholds the New Earth mission, prioritizing high quality and meaningful service to youth ages 13-25 who are impacted by the juvenile justice system, foster care system or are cross-system impacted.

Your Responsibilities:
Case Management
- Conduct intake assessment with new clients, including processing and filing intake questionnaires and narratives
- Maintain regular communication with active clients and alumni, including outreach to non-active members to check-in
- Work directly with center staff and educational staff to create an action plan that is “wrap-around” in nature ensuring the client’s success toward action plan goals.
- Complete required reports and update appropriate staff in a weekly, bi-weekly, and/or monthly basis, as needed/requested
- Always be adaptable and flexible as you will meet participants and/or families in a variety of settings including but not limited to: schools, worksites, home, court, other community-based organizations, or via digital communication
- Enforce intervention services when necessary including but not limited to restorative justice
- Actively monitor students to detect unusual or harmful behavior
- Implement Nonviolent Crisis Prevention and Intervention (CPI) strategies to prevent and de-escalate harmful behavior, reducing the risk of injury and ensuring the safety of others
- Circulates throughout classroom spaces and general areas to serve as a consistent and supportive presence to students throughout the school day
- Provides mediation in conflict situations between students and other parties to prevent behavioral crisis
- Collaborates with Special Education Team, Instructional Team, & School Admin to assist in the development, implementation, and modification of behavioral intervention and support plans
- Maintains ongoing conversations with service fidelity
Data Collection, Storage & Records
- Regularly complete download and storage protocols for all client data
- Maintain updated program software to ensure the most current protections are applied to client data
- Maintain accurate records in order to provide comprehensive services to clients
- Regularly maintain client case notes and records to be audit-ready at all times
Qualifications
Education
- MSW, MFT, PPSC, or school counseling credentials required (MSW preferred)
Experience & Knowledge
- At least four (4) years working with at-promise or system-involved youth
- Solution minded problem solver that responds quickly and effectively to any changes in the environment
- Evaluates situations and context and exercises discretion and judgment to choose courses of action
- Persuades others to consider and adopt a different point of view in a behavior crisis situation
- Ability to perform physical activities that may involve strength, flexibility and balance
- Has local organizational partnerships within Los Angeles County
- Understands the juvenile justice system and youth development
- Ability to work independently and collaboratively
- Trained in trauma-informed approaches for youth
- Trained in Restorative Practices/Restorative Justice
Interpersonal & Intrapersonal Proficiencies
- Excellent communication skills both verbal and written
- Strong active listening skills
- Excellent interpersonal skills and a collaborative management style, including the various applications of soft skills
- Able to manage crisis situations when needed
- Strategic interpersonal skills with youth
- Commitment to high professional ethical standards
- Models culturally sensitive and culturally relevant communication and interaction that respects and includes all individuals and their languages, abilities, religions, and cultures
- Excels at operating in an fast-paced, multi-tasking community environment
- Bilingual (English-Spanish) preferred but not required
