CMPAS Care Manager *Urgently Hiring* Job at Outreach Health
Under indirect supervision, the CMPAS Program Manager oversees the operational planning, establishment, execution, and evaluation of the Consumer Managed Personal Care Services Grant Program. Serves as the principal point of
representation and liaison with external stakeholders on operational matters and provides day-to-day technical/professional guidance and leadership as appropriate to the area of expertise.
The CMPAS Care Manager/Assessor is a positive leader and mentor, a torchbearer for company values, and will represent Outreach Health with integrity in the community.
Essential Job Functions
- Maintain quality service by following Outreach Standard Operating Procedures to onboard new clients timely and ensure accuracy. This includes duties such as: coordinating interest list acceptance with the local offices, timely
- communication with HHSC/reporting of acceptance rates
- Communicate effectively with HHSC to correct any client registration or authorization issues
- May maintain clients’ EHR (electronic health record) by entering new information timely and accurately
- Address client concerns timely and with empathy
- Supports other members of the Care Team to provide the best customer service possible and create an environment of trust and collaboration within the team under every service option under the CMPAS Grant including Traditional, CDS, and Block Grant
- Recruits, interviews, hires, trains, supervises, evaluates, counsels, and terminates caregivers needed to provide services for clients
- Arrange for backup services as needed for clients receiving services through the Agency model and Block Grant model
- Makes initial visits to each client’s home with the caregiver on or before initiation of service for the purpose of developing determining eligibility and co-payment amount, preparing client registration data entry forms, and assessing client needs using Form 2060 (Needs Assessment Questionnaire and Task/Hour Guide, and developing a plan of care
- Conducts visit to each client’s home at least annually for evaluation of services and reassessment of client needs; the client will be contacted by telephone as necessary to ensure services are provided as described
- Communicate with case managers, practitioners, and others as needed to facilitate client services and ensure client continuity of care
- May provide direct care to clients in the assigned caseload as needed
- Assist in the direction and coordination of quality control in service delivery
- May monitor and manage caregiver master schedule(s) and timesheets
- Assist in the overall flow of paperwork and ensure accuracy, completeness, and timeliness
- Assist with incoming and outgoing calls with kindness and professionalism
- Ensures accurate financial eligibility by calculating copayments for all clients
- Knows and follows Company policies and procedures with integrity
- Conducts all communications in an honest and professional manner
- Be a good steward of company resources, always striving to accomplish more with less and reduce fraud, waste, and abuse of funds

Required Skills, Education, Certification, License, and/or Experience
- Minimum of two years of full-time study at an accredited college or university; an individual with a high school diploma or GED may substitute one-year full-time employment in a supervisory capacity in a health care facility, agency, or community-based agency for each required year of college.

- Appreciation for the needs and characteristics of the client population
- Skills in problem-solving/interpersonal communication
- Supervisory skills to implement referred services
- Detail orientation and basic mathematical skills to follow through with payroll preparation
- Strong communication skills and ability to build a team approach with the DADS/HHSC staff
- Able to read, write, and comprehend English
- Maintain a valid Texas driver’s license, auto insurance, and own transportation.
- High attention to detail and a high degree of accuracy
- Demonstrate analytic skills when problem solving
- Proficient in MS Office Applications and ability to learn job-specific software systems

Preferred Skills, Education, Certification, License, and/or Experience
- Bilingual (English and Spanish)
- At least 2 years of relevant work experience
- Experience with EHRs as well as tracking and monitoring Electronic Visit Verification (EVV)
- Local knowledge about and connections to community health care and social welfare resources are desirable
