This position is included in a bargaining unit of Housing Works’ New York City employees represented by a labor union known as the Retail Wholesale and Department Store Union (“RWDSU”). Accordingly, the RWDSU has the exclusive right to bargain over the terms and conditions of employment related to this position and this position may become covered by the terms of a collective bargaining agreement (a “CBA”) between Housing Works and the RWDSU

Compensation Range: $45,000 - $50,000 Commensurate with experience

Program Overview:

CONNECT is a community-based program that expands the capacity of existing licensed OMH /Article 31 mental health programs by engaging the clinic’s
surrounding community in the development and design of enhanced services. Programming under CONNECT involves ongoing community outreach and collaboration with the immediate geographic areas associated with each
identified site, and with that information, the creation of innovative programming aimed at reducing inpatient psychiatric hospitalization among individuals living in the community. The program seeks to address perceived norms
around mental illness and build interventions that are more responsive to individuals and community’s needs.CONNECT seeks to empower through education, intervention, and connection. This is a groundbreaking initiative that can shape the way community-based mental health services are delivered. Each team member in their
respective role is charged with providing psychoeducation, resources coordination services which includes individual and group-based interventions. CONNECT is currently seeking to fill a Community Liaison role in East New York.

Essential Job Functions:The Community Liaison works under the supervision of the Program Manager for Community Strategy to provide community engagement and outreach work associated with CONNECT at the East New York Health.
This role entails building community presence through canvassing and tabling and other community engagement practices to promote CONNECT services and engage community in conversations for self- reported health and wellness status, experience in receiving services and identified needs for support.
Community Liaison will recruit participants into the program and build strong relationships with residents, businesses, and community groups.
The Community Liaison will hold forums geared toward obtaining feedback and information specifically focused on patient service needs and care gaps present within the community.
The Community Liaison is a self-starter who can successfully be part of a
multi-function and interdisciplinary team and comfortable with managing many diverse tasks.
The ideal candidate has lived experience and/or interactions with the justice system and other systems (i.e., housing, foster care etc.)
and an understanding of how to navigate them; and has in-depth knowledge and familiarity with the community (East New York or East Harlem) and the human services landscape of that community.
This role offers capacity building for a trauma-informed, holistic, and culturally competent practice.
The Community Liaison should be committed to the framework of harm reduction in their practice as well as a strong interest in developing innovative, holistic, and empowering interventions.
The Community Liaison presents as a brand ambassador to uphold and advance the values, ethics, and overall mission of Housing Works with integrity.

Other Functions: The Community Liaison works closely with the clinical and case management teams to coordinate and facilitate interventions that are responsive to the specific interests and needs of the individual, which may include connection
to treatment, psychoeducation, addressing trauma, harm reduction support and identifying concrete needs.
The Community Liaison may assist in tracking clients identified as higher risk to keep them engaged as well as engaging with their families or those around them and connecting them to supportive services.
Related duties also include conducting short screening assessments and completing program enrollment.

Responsibilities:
1. Provide ongoing community engagement at identified site (either Park Ave./Bailey House or East New York)
2. Play a key role in prospecting partnerships with community stakeholders, setting up meetings to assess care
gaps and needs of the community, and developing forums for information gathering to ensure community
input is included.
3. Participate in quality improvement projects to ensure community feedback is considered when making
changes/improvements to programming.
4. Works in collaboration with Program Manager for Community Strategy to use community feedback and
observed trends to propose programming ideas aimed at reducing barriers to treatment for those in the
community.
5. Engage residents in assigned geographic area and help disseminate awareness/education materials and
information about community events and important resources.
6. Collaborate with case management and clinical teams on client engagement planning.
7. Document all contacts with high-risk individuals.
8. Attend and help facilitate community events
9. Assist in the creation of public information materials to keep the community informed of the program’s
services and community-based initiatives.
10. Attend, participate and complete all program trainings.
11. Maintain routine communication with the larger team and assist with coordinating clients interventions with
appropriate agency staff
12. Creates effective presentations with the assistance of Program Manager for Community Strategy to share
programming with community members.
13. Works collaboratively with the agency’s Community Partnerships program to support CONNECT initiative
and enhance referral networks for treatment services.
14. Maintain the integrity of the programs through monitoring of regulatory standards, codes of conduct, and
ongoing HIPAA compliance.
15. Responsible for data collection, documentation and administrative reports, and other tasks as assigned by
the Program Manager for Community StrategyJob Requirements:Bachelor's Degree in human services field or equivalent preferred. May consider extensive and related work
experience in lieu of degree. Experience in outreach and community engagement (as staff or as volunteer);
experience in community organizing a plus.
Strong organizational, written, and public speaking skills
Experience in the care and management of persons living with acute/chronic health service needs and/or
HIV/AIDS related illnesses preferred; experience working with those living with mental health needs required.
Personal qualities of warmth, empathy, and sense of responsibility are essential.
Computer literate with proficiency in Microsoft Office.
Ability to work in the field as well as on-site.
Available for a flexible schedule.
