SUMMARY:

The Care Services Coordinator will provide comprehensive and professional services, community resources, and disease management for people with ALS, their families, and caregivers. Acts as a liaison for patient/family with healthcare personnel and community resources to ensure necessary care is provided promptly and effectively. This position reports to the Director, Care Services, and will be based in the Nashville, TN.

  • This is currently a hybrid position with remote work as well as travel to clinic or other locations for direct services. This position serves our central Alabama area.

ESSENTIAL DUTIES AND RESPONSIBILITIES.

Core duties and responsibilities include the following. Other duties may be assigned.

Program Delivery and Measurement – Performs routine to moderately complex care management activities and patient advocacy across the continuum of care to facilitate and promote quality of life and minimize fragmentation of health care delivery. Manages an assigned caseload of North Arkansas constituents to include, but not limited to, developing care plans, home assessment visits (at times virtual), ALS clinic support, outreach, and enrollment of clients.

  • Performs patient and family centered assessments, clinic visits, clinical coordination, family education conferences, advocacy, and other related areas of need.
  • Evaluates the constituent to identify limitations which impede daily living; record case notes on a timely basis; develop care plans and advocate the coordination of social/health services where needed.
  • Identifies short and/or long-term needs of people living with ALS and their families using established programs, policies, procedures, and guidelines.
  • Works collaboratively as a vital liaison/team member within the ALS Association, local and home office staff, and ALS Multidisciplinary Clinics/Treatment Centers to ensure effective communication and continuity of care for patient services.

Support Groups – Plans and facilitates monthly support group(s) for clients/families; occasionally provides educational portion of monthly meeting; arrange for professional/expert topic speakers. Responsible for growth and function of support group(s).

Public Education and Awareness – Participate in organization/coordination of annual statewide educational program/symposium. Facilitate professional/educational in-service programs as necessary, acting as a resource to the community and other staff members. Collaborate with Development Team in promoting the Walk to Defeat ALS, as well as attend key special events.

Administrative Functions – Ensures that all materials provided and utilized are in accordance with ALS National policy and best practices; maintains confidential documents and computer-based files of constituent information and retrieval system for interrelated files and records; prepares regular reports of activities; ability to format and proofread detail-oriented letters, memoranda, and reports using Microsoft Office programs.

Actively promotes the Chapter’s mission, services, programs and events in the community. Other duties as may be assigned.

QUALIFICATIONS:

  • Bachelor’s Degree, in social work, health administration, public health, nursing, or a related area required.
  • Minimum of three years experience in a human services organization preferred.
  • Demonstrated ability to utilize skills such as problem solving, solid reasoning and critical thinking.
  • Ability to use computer effectively and efficiently including MS Word, MS Excel, MS Outlook, and database program as appropriate.
  • Must have a valid driver’s license, valid auto insurance and reliable transportation to meet service area demands.
  • Must be able to use independent judgment as well as work collaboratively with staff.
  • Ability to travel within the state to meet with patients, families and health care providers as required or requested.
  • Ability to travel outside of the state on occasion and as needed/required.
  • Commitment to organization, accuracy, attention to detail, highly efficient and conscientious about thorough follow up.
  • Skilled communicator, with superior interpersonal, verbal and written skills.
  • Ability to work under and comply with continuous and multiple deadlines evolving priorities.
  • Ability to occasionally work some evenings and weekends as required.
  • Demonstrates professional maturity, the ability to keep information confidential when needed, excellent leadership qualities and compassion for those we serve.

PAY TRANSPARENCY:

The ALS Association’s pay range for this position is $24,635.21 - $30,718.60 annually.

The ALS Association has compensation ranges for various work locations throughout the United States, allowing us to compensate employees competitively and consistently in diverse geographic markets. The above-listed listed range shows prospective compensation for this role; the appropriate range will be established by what is eventually determined to be the candidate's primary work location. Various factors, including the complexity and significance of the role, job duties/requirements, relevant experience and abilities, and primary work location, determine individual compensation. Compensation ranges are evaluated and typically adjusted on a regular basis. Offers are made within the compensation range that is in effect at the time of the job offer.

The Association may, at its discretion, offer merit-based increases tied to individual performance. The benefits listed may differ depending on employment status with the Association.
