Job Summary: The pharmacist delivers patient-centered pharmaceutical care in an accurate, timely and courteous manner. Patient-centered pharmaceutical care includes involvement in all areas of the Medication Management process including selection and procurement, ordering and transcribing, preparation and dispensing, storage, administration, and monitoring. The focus of the pharmacist is to ensure safe medication practices that improve patient outcomes.
Demonstrates Competency in the Following Areas:
- The pharmacist monitors patients’ drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose.
- The pharmacist assists the professional staff with drug dosing (including pharmacokinetic consults) when requested following appropriate policies and/or guidelines where available.
- The pharmacist utilizes available patient information to guide recommendations regarding appropriateness of medication therapy considering patient specific factors (age, disease state, organ function), recommended dosage and potential complications
- The pharmacist provides clinical consultation and clarification to practitioners, suggesting cost-effective therapeutic alternatives to medical staff where appropriate
- The pharmacist provides accurate, adequate and timely drug information to the professional staff upon request.
- The pharmacist provides drug information to patients and their families using appropriate terminology and language based on patient’s age and comprehension
- The pharmacist demonstrates ability to adjust doses based on patient’s age utilizing appropriate age specific references.
- The pharmacist sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection
- The pharmacist interprets medication orders (verbal, written and electronic) and transcribes the orders to computerized patients medication profiles accurately
- The pharmacist maintains accurate, complete patient medication profiles including known medication allergies and other relevant patient data as pertinent (e.g., height, weight, concurrent disease states, etc.)
- The pharmacist accurately checks technician work.
- The pharmacist compounds and dispenses pharmaceuticals accurately and efficiently utilizing appropriate aseptic technique for sterile products.
- The pharmacist handles controlled substances in a manner that adheres to state and federal laws.
- The pharmacist documents all clinical activities and interventions accurately and completely.
- The pharmacist participates in the quality improvement and medication use review activities of the department and hospital. Collects data; conducts quality monitors and inspections; and maintains logs, records, and other documentation as assigned.
- The pharmacist appropriately directs and assists technical staff in completing day-to-day operations.
- The pharmacist accepts and completes delegated tasks and special projects for the department completing work within the accepted time constraints.
- The pharmacist is flexible in resolving scheduling conflicts and periods of shortages in staffing.
- The pharmacist is punctual and dependable; reporting to work as scheduled. Absenteeism and tardiness are within hospital standards
- The pharmacist demonstrates ability to work cooperatively with all staff
- The pharmacist maintains a neat, professional, well groomed appearance observing the pharmacy dress code and wearing identification badge at all times.
Professional Requirements:
- Must be licensed by State of Arkansas State Board of Pharmacy
- Must have a bachelor’s degree or Doctorate in Pharmacy from an accredited college of pharmacy.
- Previous hospital experience or ASHP-accredited residency is preferred but not required
