Reports to Administrator Paula Burroughs
Job Summary: The primary purpose of this position is to coordinate the admissions of new and returning residents from home, hospital, or other health care settings and to maintain the facility at its maximum capacity. The position also entails a marketing component to hospitals and other potential clientele for the purpose of promoting long term, transitional, and exceptional care offered by the facility.
Must be physically and mentally capable of performing routine job duties.
Be personable with residents, families, and staff in a professional and cooperative manner.
Must have compassion, tolerance, and understanding for the residents of the facility.
Good organizational, planning, and marketing skills.
Knowledge of hospital discharge planning and policies as it relates to the long term care population.
Knowledge of JCAHO, OBRA, IDPH, and HFS regulations relevant to admissions, discharges, and resident rights.
Supervisory Responsibility Supervises Social Worker for LTC Unit
Experience: 1+ years experience in skilled nursing facility. Must have LSW or CSW
