Description:

Haymarket Center is seeking a Family Therapist.

This position is responsible for clinical assessing, treatment planning and treatment to mentally ill homeless patients who may have a concomitant substance abuse disorder and to their family members.

Facilitate family therapy sessions and document in client chart;

Clinically assess and re-assess clients and their family members to identify relational issues that may be exacerbating the clients mental health issues and triggering the client to re-engage in unhealthy coping behaviors including but not limited to substance use;

Provide crisis intervention as required and follow-up clinical assessment with patient(s) involved to determine need for appropriate clinical course of action;

Facilitate didactic and therapeutic groups related to healthy communication in interpersonal relationships;

Facilitate didactic and therapeutic groups related to healthy parenting skills;

Assist clients in communicating pertinent information to entities including but not limited to DCFS who may also be providing services to the client;

Provide continued care/aftercare planning to assigned clients and document in client chart;

Maintain assigned client charts in manner which meets Haymarket Center quality assurance standards and is clear in its documentation of clients treatment progress;

Provide case management, referral and referent contact services to assigned clients and document in client chart;

Monitor physical and emotional health status of patients and facilitate appropriate referral when assessed to need medical or psychiatric evaluation and/or care;

Attend patient/staffing interventions and document patient chart;

Attend weekly supervision with assigned supervisor;

Complete monthly PPW reports and submit to state representative of PPW program.

Other duties as assigned.

Requirements:

Qualifications will include:

A Bachelor's degree minimum (Masters preferred) in human service related field;

Clinical licensure (LPC, LCPC, LSW LCSW); and/or CADC preferred. Credentialing required within two years of hire date;

Knowledge and the ability to perform all Core functions;

Familiarity with mentally ill and homeless populations;

One year paid work experience in a clinical role;

Two years prior MH experience;

Foundational knowledge regarding mental health issues, therapy and substance abuse 12 step philosophy;

Excellent verbal and written communication skills;

Basic Computer skills to include typing and Microsoft Office;

Committal to attend homeless and MISA related workshops;

Other duties as assigned.
