POSITION SUMMARY:
Under the supervision of the Community Health Coordinator, the Health Coach is involved in all medical membership components including but not limited to r eferral, o nboarding proc ess, pre/post data measurements and transition to self-directe d health and wellness programs.
They are responsible for ensuring participants ha ve a welcoming and engaging experience that supports their health and wellness goals.
The Health Coach is responsible for performing those functions in accordance with the stated mission, values, principles, and practices established by the administrative staff, governing committees and Board of Directors of the YMCA of the Triangle Area.
This position support branches in assigned Region. Regions include:
South West Triangle —Chatham YMCA, Ingram Family YMCA
ESSENTIAL FUNCTIONS:
Models relationship-building skills in all interactions. Develops and maintains collaborative relationships with community organizations. Maintains regular, clear, and concise communication within area of responsibility.
Audit s all evidence based programs held throughout branches and community at a minimum of 2 times throughout program duration.
Report s all pre and post data evaluation on patient outcomes to Community Health Coordinator.
Ensures program instructors are entering data in timely and correct manner.
Ensures program standards and fidelity standards data measurements are met and clinical outcomes are met with anticipated program outcomes.
Ensure s all evidence based programs are tracking all the applicable dat a points for each participant. Data system must aggregate data in order to share with key stakeholders .
Support s instructors to ensure participants have an engaging, meaningful experience, meet health and wellness goals.
Attend s trainings and meetings as required.
Supports implementation strategies for future evidence based programs
Work s with compliance officer to ensure the program and staff adhere to YMCA of the Triangle’s HIPAA policies and procedures.
Responsible for other op portunities as assigned by Community Health Coordinator.
QUALIFICATIONS:
Bachelor’s degree in Exercise Science, Health Education or Public Health is preferred and/or 2-3 years of experience working in the community health, healthcare or exercise fields.
Strong interpersonal skills, including written and oral communication, decision-making and conflict resolution.
Must possess strong facilitation skills, and have the ability to work autonomously as well as collaboratively with a team.
Ability to relate effectively to diverse groups of people from all social and economic segments of the community.
Professional, genuine, nurturing, welcoming, courteous, friendly and relates well to staff and customers by providing excellent customer service.
Comprehension of social determinants of health, share empathy, and work with health seekers to meet their health and wellness goals.
Must have a passion for helping others achieve their healthy lifestyle goals. An understanding of behavior change is required.
Must be able to support and teach at least 2 evidence based programs .
Basic computer skills, including Microsoft Office applications.
Multi-lingual desired but not required.

Compliance with all COVID related PPE, cleaning, reporting and prevention expectations efforts. Further training and detail will be provided upon question and/or hire.
