Position Summary

The Digital Health Ambassador is our liaisons to our patients. The Digital Health Ambassador is responsible for providing in-person technical assistance to our older adult patients with the goal of increasing overall digital literacy, including but not limited to MyChart enrollment and engagement and enrollment and usage of remote blood pressure monitoring. The Digital Health Ambassador is responsible for actively engaging our patients while providing high quality customer service.

Duties

• Assist our patients in enrolling into our patient portal.
• Educate and assist our patients with navigating MyChart so they are familiar with accessing features such as reviewing their labs, communicating with their clinical team, scheduling appointments, and accessing their visit summary.
• Assist our patients in scheduling appointments via our patient portal.
• Enroll patients who meet necessary criteria into the remote patient monitoring program.
• Develop, track and report on monthly goals, e.g., total patient interactions, number of MyChart activations, direct appointments scheduled, total patients enrolled in remote patient monitoring, etc.
• Effectively maintain and protect the confidentiality of all medical records and patient information.
• Collaborate with the MyChart Support Specialist to create/update patient education materials pertaining to MyChart features.
• Provide patients instruction on how to compose and send messages to clinical staff based on patient requests or as needed.
• Answer patient incoming inquiries in a polite and professional manner.

Requirements

• High School Diploma or equivalent.
• At least five (5) years of patient support or customer service experience.
• Intermediate comfort level with digital technologies on various platforms, such as PC and mobile devices.
• Bilingual in English and Spanish Required.
• Experience with data entry and Microsoft Office applications, including Outlook, Word and Excel.
• Computer literacy working with PC-based software applications and proficiency with the Microsoft Office suite, including Outlook, Word, PowerPoint, and Excel.

Desired

• 
  Associate degree in business administration, healthcare administration, or related field.
• Knowledge of medical terminology and triage procedure.
• At least two (2) years’ experience working with computerized scheduling systems.
• Knowledge of OCHIN Epic
• Knowledge of Computer software updates

If you are hired at TrueCare, your final base salary (within the pay range), will be determined based on factors such as education, knowledge, skills, and experience. Please keep in mind that the range mentioned above is the full base salary range for the role.
