Provide all aspects of coding, chart organization, transcription & administrative reports.

  • Maintain medical records and medical records computer system hardware & software. Oversight of completion of records, audits and reports within compliance of regulatory guidelines, and as requested.
  • Maintain master medical records log utilizing established computer data entry system.
  • Assure that all applicable HIPAA regulations are followed in the transmission of patient information.
  • Responsible for the process to assure all physician documentation requirements are met and to be in compliance with Joint Commission and all regulatory bodies.
  • Ensure that health information is provided in a timely manner.
  • Code to achieve maximum reimbursement while maintaining compliance with coding guidelines, rules, and regulations.
  • Serve as primary source/person to answer questions or issues regarding coding.
  • Develop reports and business correspondence as needed and/or required.
  • Prepare and/or present reports concerning documentation, timeliness of records processing, and documentation/reporting.
  • Maintain Joint Commission/regulatory body readiness as it relates to medical record documentation compliance.
  • Responsible for the submission of regulatory data.
  • Assure all applicable HIPPA regulations are followed in the transmission of patient information.
  • Compliant with policy, prepare and review all requests for patient information.
  • Strive to improve efficiency and reduce costs within assigned department.
  • Attend weekly staff meetings and monthly committee/management team meetings, regulatory activities, etc.
  • Lead/organize the utilization review committee.
  • Coordinate and facilitate meetings as required.

GENERAL ACCOUNTABILITIES

  • Know and comply with Hospital’s Medical/Clinical Governance standards, policies and practices.
  • Support company directives, vision, and goals.
  • Work collaboratively with all facility personnel.
  • Provide a point of contact for leadership to evaluate care delivery and review quality measures. Support leadership team to assure clear communication occurs across the health care continuum to support patient/family needs.
  • Support the leadership team to assure the staff maintains accurate records, utilizing phone dictation systems and computer systems where appropriate.
  • Work with the nursing team and others in developing new roles and responding the changing healthcare needs.
  • Monitor the medical record department’s efficiency and effectiveness of services.
  • Monitor the effectiveness of clinical practice through the quality assurance strategies as presented by hospital leadership.
  • Keep up to date with pertinent health-related policy and work to consider the impact and strategies for implementation.
  • Assure medical records follow consistent chart organization, transcriptions with signatures, and scan to billing service in a timely manner, etc.
  • Assure health information is readily available as needed by appropriate requestors.
  • Effectively present information and respond to questions from superiors, clients, and the general public.
  • Responsible for accessibility of appropriate hospital paper forms and electronic equipment where appropriate.
  • Write reports and business correspondence as needed/required.

Preferred Education/Experience/Qualifications/Certifications

  • Bachelor’s degree
  • Current certification as a RHIT Certification OR as a Registered Health Information Administrator [RHIA]
  • Previous experience with regulatory body surveys/inspections
