Need a RN who enjoys working with patients and families in their home to teach about medications, self-management of disease process and instructing patient and caregiver how to infuse IV's, care of central lines, chemotherapy disconnects, foley care, pleurix care, wound care. If you enjoy coordinating care with physicians, other team members, and referral sources. You will help patient prevent re-hospitalizations and promote individualized patient care goals. Must be RN licensed in Illinois and willing to cover West and Northwest suburban areas: Oak Park, Melrose Park, Naperville, Batavia, Elgin, Schaumburg, Carol stream. Glen Ellyn, Lombard.
