Hospital at Home Nurse Practitioner or Physician Assistant

The Geriatric and Palliative Care Team at Kent Hospital in Warwick, RI is seeking an experienced nurse practitioner or physician assistant who is familiar with home care, enjoys providing care in the home setting and has experience in caring for patients with complex medical needs to join our Hospital at Home Program providing acute care in the home setting.

About the Opportunity:

• Full-Time Opportunity
• 100% clinical home visits and/or tele-video visits to provide care to acute medically complex patients
• Program based out of Kent Hospital in Warwick, RI with patient homes within the hospital’s region
• Join team of physicians and nurse practitioners
• 5-6 patients per day
• Predominantly in-home visits with some tele-video
• Average Patient Age: 65+
• Schedule: Mon – Fri 8:00am – 5:00pm with weekend rotation
• Call dependent on team schedule, which is flexible
