Responsibilities:

• Coordinate with the CCM for individual monthly schedules
• Ensure staff has the necessary skills and tools to meet the individual’s needs.
• Random visits to the homes to ensure staff is doing their job duties and are in proper uniform.
• Maintain a safe, healthy, positive environment for the individuals we support.
• Schedule necessary appointments for the individuals.
• Ensure staff is recording data for each individual we support.
• Initial and ongoing contact with families
• Plan appropriate meals and ensure grocery shopping is completed.
• Conduct bimonthly staff meetings.
• Complete required reports; Quarterly, Semi-Annual, Annual, incident/accident, etc.
• Orientation of new staff to site specific training.
• Keep the director and CCM informed
• Perform all duties of the HM while working with individuals we support.
• Covering shifts that the CMM can't cover
• Other reasonable duties as assigned.

Requirements:

• 18 years of age or older.
• 1 Year Patient Care Experience as Home Health Aide - HHA, PCA, or caregiver, preferred, but we are willing to train the right candidates.
• Must have a reliable source of transportation, valid driver’s license for 2 years, automobile insurance if driving while on the job or seeing multiple clients per day. We do have clients that require no transportation.
• Must be able to pass criminal and driving background, and drug screen.
• Must be available to work some weekends/holidays.
• Highschool Diploma or GED
