Seaside Occupational Therapy, LLC is hiring Speech Therapist's for Outpatient and Home Health services!
The Speech Therapist has the following responsibilities:
- Assists the physician in evaluating the patient's level of function. Identifies functional speech deficits and establishes goals and plan of care to improve patient' s function, revising as
- Performs and complete re certifications, resumption-of-care, and discharge Comprehensive OASIS assessments and associated paperwork in accordance to agency policy.
- Instructs patient and family members, as well as other health team members, in methods of assisting the patient in improving communication
- Recommends communication devices/aids as
- Periodically re-evaluates and upgrades the patient's program to maximize functional improvement. Records and reports to the patient's physician the patient's progress in treatment and any changes in the patient's condition and plan of care.
- Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient
- Performs and submits required documentation of patient's evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders. All patient visit notes must be turned in the next scheduled workday, not to exceed forty-eight (48)
- Maintains necessary statistical and billing
- Keeps abreast of changes in speech pathology practice and theory, contributes to the field through research, presentations, and in-services.
- Keeps his/her immediate supervisor informed of problem areas and makes suggestions for
- Attends and contributes to in-services, clinical records reviews, meetings required to ensure coordinated and comprehensive plans of care for the patients of the
- Complies with Homecare Agency Compliance, Infection Control Policies, and HIPAA Policies.
Requirements:
- Must hold a Master's Degree in Speech language Pathology (may be dually certified in audiology.)
- Must hold a valid state license (where appropriate.)
- Must show evidence of a current satisfactory physical examination and/or evidence of freedom from communicable diseases, where required by the state.
- Must be capable of easily lifting 30 pounds of dead weight alone.
- Must have the ability to walk, bend, stand and reach constantly during at least an 8 hours day/shift.
