Position Summary
:

Under general supervision of the Clinical Manager, facilitates, schedules and coordinates assignment of all referrals to the appropriate case manager. Participates in authorization process. Assumes responsibility for processing of referral documentation and entry of data into the electronic system. Schedules all home visits for the appropriate disciplines in a timely manner and enters schedule changes into the electronic scheduling system.

:

  • Maintains a safe, clean working environment, including unit-based safety and infection control requirements.
  • Receives patient referrals from hospital, physician’s office and community. Completes intake screening including obtaining and reviewing all required information to ensure referral meets home health admission criteria. Notifies Director and/or Clinical Manager with referrals that are not complete, do not meet admission criteria, or require further review and direction on whether referral meets home health admission criteria.
  • Schedules all new referrals to the appropriate clinician based upon location of patient, clinician availability, and skill set as directed by Clinical Manager.
  • Performs scheduling activities that facilitate agency operations. Enters referral data and clinician schedule changes and reassigns patients appropriately into electronic scheduling system.
  • Reports any scheduling changes and/or patient, staff concerns to Clinical Manager.
  • Effectively communicates patient information and/or schedule changes to appropriate staff as needed.
  • Manages multiple tasks effectively under pressure and possess detail orientation to ensure accuracy of referral and scheduling.
  • Receives and responds timely to Intake and Scheduling requests from internal and external customers.
  • Maintains referral data logs and provides follow-up for hospitalized and long-term care patients, ensuring subsequent agency referral.
  • Assists RN in providing skilled nursing visits to patients and follows plan of care developed by team members.
  • Participates in continuous quality improvement processes related to this patient population.
  • May perform other duties as assigned.


Qualifications:

  • Minimum of one (1) year acute care, preferred.
  • Knowledge of medical terminology.
  • Good communication/phone skills.
  • Computer skills, required.


Licenses:

  • Current California LVN license, required.
  • Current California Driver’s License, required.
  • Current auto insurance, required


Certifications:

  • Current BLS required upon hire; American Heart Association Healthcare Provider.


Each new hire candidate who is offered employment must pass a physical evaluation, urine drug screen and pre-employment background checks before starting work.


  • Salary/Hourly Wage range based on experience.
