IVAS Pharmacist - Santa Monica Hospital Job at UCLA Health
The Staff Pharmacist performs a range of functions, covering multiple pharmacy locations, units or other settings during a variety of shifts, including but not limited to float shifts. Staff Pharmacist duties include: * Preparation and dispensing of drugs in all inpatient settings * Review and verification of medication orders and patient medical records * Optimizing safe medication therapy * Review of patient medication profiles * Counseling patients on appropriate drug use * Interacting and/or rounding with other health professionals to help provide safe and effective use of medications * Provision of clinical services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, TPN and amino-glycoside monitoring, pharmacist interventions) to neonatal, pediatric and adult patients according to departmental policy and procedures * Oversee dispensing, return and waste of controlled substances on assigned shift/unit * Complete operational and clinical projects, as assigned by Pharmacy Management * Work in assigned centralized and decentralized Pharmacy areas and patient care units Schedule: varied shifts, depending on department needs, including weekends and holidays Hourly Salary Range: $80.94 - $94.81
We’re seeking a dedicated clinical professional with: * Pharm D. degree and current CA Pharmacy license (required). * ASHP-accredited residency (preferred). * Inpatient pharmacy experience (highly preferred). * Exceptional communication, interpersonal, organizational and follow-through skills. * Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations. * Ability to compound intravenous admixtures and other sterile products.
