The Position

We are looking for Per-Diem LPN/RN's to provide in home skilled nursing visits in the greater Franklin County area (Greenfield, Athol, Orange, etc.). Visits are usually repeating at intervals and most often include medication management (medication dispenser or planner refills), well being checks and/or vital signs checks, or care management. Appropriate documentation of visits occur on our electronic medical record program WellSky/ClearCare. Per diem nurses are compensated at a rate of $45 per visit.

Other job responsibilities may include:

• One on one patient care in home or facilities as guided by the care plan and the Director of Nursing or their designee
• Client assessments/intakes
• Complete quarterly field staff supervisions on home care aides
• Provide assistance and direction to problems encountered by both office and field staff
• Assess needs of clients and abilities of field staff to ensure best possible care for clients
• Coordinate case management with Director of Nursing and/or Office Manager.

Minimum Qualifications

• Must possess current LPN/RN License in Massachusetts and possess the knowledge of nursing theory and practice and the skills to provide competent patient care
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Minimum two-years experience in the health care field; home healthcare preferred
• Must be fully vaccinated or be willing to complete full vaccination by date of hire and proof of vaccination will be required
• Must possess a reliable cell phone, access to email, and ability to download our EMR for use on your cell phone
• Ability to maintain a professional relationship with co-workers, home care aides, clients and family members, as well as other health care professionals and management.
• Must be able to respond to emergencies in a calm manner.
• Must have great attention to detail and proper documentation skills
• Position requires full range of body motion including handling and lifting patients, manual and finger dexterity, and eye-hand coordination. Requires standing, walking, and the ability to ascend and descend stairs
