Purpose:

• Provides nursing care to the hospice patient, family and caregiver, as directed by the supervising Registered Nurse (RN) and Nursing Supervisor, in compliance with Medicare Hospice Conditions of Participation, New Jersey licensure laws and agency policies and procedures. Performs timely documentation of patient services based on the patient’s plan of care. Works closely with other members of the Interdisciplinary Team (IDT) to ensure patient and family care needs are being met.

Responsibilities:

• Ensure the quality and safe delivery of hospice services to patients/families/caregivers.
• Implement the patient’s plan of care as directed by the hospice RN.
• Provide nursing care by utilizing all elements of nursing process as directed by the hospice plan of care.
• Responsible for checking and administering medications according to New Jersey Nurse Practice Act.
• Assess and provide patient/family/caregiver education and information pertinent to diagnosis and plan of care.
• Participate in coordination of hospice services, appropriately communicating and reporting the need for hospice IDT member intervention to the RN or Nursing Supervisor.
• Provide appropriate pain/symptom management and evaluate patient’s response to treatments/medications.
• Submit accurate and relevant clinical notes regarding patient’s condition and care given.
• Observe patient for changes in condition and report changes and patient’s, family’s and/or caregiver’s needs to hospice RN or Nursing Supervisor.
• Assist patient/family/caregiver in self-care techniques as appropriate, working in conjunction with other hospice interdisciplinary team members.
• Assist patient/family/caregiver and other team members in providing continuity of care.
• Provides continuous care services to patients as assigned and required.
• Work in cooperation with family/caregiver/and IDT members to meet the emotional needs of the patient/family/caregiver.

Education/Experience:

• Graduate of accredited practical nursing program.
• LPN licensed in the State of New Jersey.
• Has minimum of one (1) year general nursing experience. Community health/hospice or medical/surgical experience is preferred.
• Ability to work within an interdisciplinary setting.
• Understanding of hospice philosophy and needs of terminally ill.
• Valid driver’s license and reliable transportation that is insured in accordance with New Jersey requirements.
