Responsibilities:
Under the direction of the Director of Pharmacy, the Medication Reconculiation LVN will be responsible for the collection and electronic documentation of medication and allergy histories of newly admitted patients.
Provide support in obtaining, verifying for accuracy, and recording initial medication histories from patients, theis designated family/ caregivers and/ or transferring facility.
Qualifications:
Education, Expereince, Training
1. 1 year expereince in Pharamcy
2. State Lvn License / State Certficate Pharamcy Tech License
3. Starting in Janaury 2013, all new hires require high school dipolma or equivalent.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $28.00 to $37.68. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
