JOB DESCRIPTION:

ANESTHESIOLOGIST NEEDED IN FLORIDA

Coverage Type:

DAY SHIFTS

ALL CASES SEEN AT LAWNWOOD: ORTHO, TRAUMA, NEO, PEDS, CARDIAC, UROLOGY, DENTAL, PLASTICS, GENERAL

· EMR- IPRO

Average credentialing time- 90 days

SUBMISSION REQUIREMENTS

· Must have FL license

· Must be Board certified

· Clean malpractice file and background

AS A REMINDER:

· All candidates must have met the minimum shift availability listed on the job.

· CV must have all dates in MM/YY to MM/YY format

· Work History must be in Sequential order with most resent-on top

· Any Gaps over 30 days must be explained on the CV

· EPA (Please use the attached EPA. It’s also in Trio under your Name & then click on Help)

· All Licenses, Boards and Certs with expiration dates must be listed on CV

· Anyone with malpractice must provide a NPDB

· Anyone with legal issues must provide supporting documents

Availability

· 1st possible date they would be available?

· How many shifts would they be able for per month?

· What month is their schedule completely open?

· Any shift preferences (Day, Night, Blocks…etc)?

· Any dates in the next 3-6 months they can’t work?

2- Locum - Physician - Anesthesiologist Okeechobee, FL

JOB DESCRIPTION-2

Reason for need: Coverage

Length of need: Ongoing

Average Time to Credential: 90 days

Majority Case Type: Gen Surg, GI, GU

MD - 80% supervision 20% doing own cases

Operating Rooms: 2 ORs plus GI suite

Life Support Requirements: ACLS, BLS

Shift Days: Monday thru Friday Days

Shift Times: 7am starts

Call Required: YES (1-2 weeknights per week, can do more if they want)

Weekend Required: YES (1-2 weekends per month, can do more call if they want)

Current Available Shifts: MD will work full time Monday thru Friday, will work 8hour days 7am to 3pm, OT over 8 daily.

All Call is Beeper Call, no in house. Call is absolutely required
