A substance abuse treatment facility located in Fresh Meadows, Queens New York is seeking full time, part time and per diem LPN's. The LPN assist the Registered Nurse in the delivery of nursing care and assist in patient teaching assignments. Positions available for evening and night tours. 7 hour and 12-hour shifts available.

Responsibilities include, but not limited to:

• Assists in providing nursing care to patients through use of the nursing process.

• Collaborates with other disciplines and departments to facilitate patient care.

• Assists in assessment of physical, psychological and social dimensions of patients admitted to the Chemical Dependency Unit.

• Assists in planning a multi-disciplinary plan of care for patients.

• Assists in and directly participates in implementing plan of care.

• Implements clinical and technical aspects of care and nursing orders in accordance with established policies and procedures. Intervenes with proper technique, procedure and safety precautions to meet individual needs of patients.

• Implements medical plan by administering medications and documentation of plan of care.

• Functions within limits of State Nurse Practice Act and nursing department policies and procedures.

• Assists in evaluating nursing care provided, and the effectiveness of this care.

• Performs all duties under the supervision of a Registered Professional Nurse.

• Observes patients and records information obtained in the initial nursing assessment and uses this information to assist in the formulation of the treatment plan.

• Documents nursing intervention, patient response, effectiveness, complication, etc., and communicates information to peers and physician.

• Keeps medical and other personnel who have a need to know informed regarding changing conditions of patients and/or pertinent observations.

• Teaches appropriate self-care to patients as needed.

• Demonstrates ability to solve problems in a logical, appropriate and professional manner.

• Respect and support the patients’ rights and confidentiality.

• Maintains a professional demeanor at all times.

• Will assist in all aspects of patient care, including body searches when indicated.

• Perform other job assignments as delegated or required.

Required:

• Must be licensed as a Practical Nurse in the State of New York and possess a current registration.

• Certification in CPR

Must be computer literate.
