Manager, ACP Facilitation, Remote - Bilingual Spanish Preferred Job at Aledade

Iris Healthcare, powered by Aledade, Inc. is looking for a full-time Bilingual Spanish Licensed Social Worker (LSW) or Registered Nurse (RN) with a background in Advance Care Planning (ACP)/End-of-Life Care, to join our growing team. The Manager, Facilitation oversees a team of approximately 8 Facilitators who are providing Advanced Care Planning to Patients. The Manager serves as the first point of clinical escalations.
Our interdisciplinary team walks the member through every step of the advance care planning process, from exploration of values through distribution and revision of necessary documents. The primary role of the Facilitator is to guide members through this conversation and document their preferences. These conversations include providing education on the patient’s primary medical condition and on such medical interventions as feeding tubes, ventilation, and CPR. Member encounters are conducted remotely via telephone.

Responsibilities:
- Recruits, interviews, hires and trains new staff
- Oversees the daily workflow of the department
- Provides constructive and timely performance evaluations
- Handles discipline and termination of employees in accordance with company policy
Essential Duties and Responsibilities:
- Oversees and provides clinical and operational support to the Team which includes, but is not limited to, QA (calls and documentation); 1:1 and Group/Team Supervisions; Coach to and Manage quality and performance metrics; and Evaluations of direct reports.
- Provides valuable operational feedback (i.e. Trends/gaps) and collaborates on solutions with Asst. Directors and/or Director
- Participate in functional area development/problem solving through special initiatives and projects as needed by Organization
- Lead special Teams or Projects
- Other duties as assigned
Required Skills/Abilities:
- Clinical Subject Matter Expertise:
- Strong clinical background related to CACP and ability to make independent clinical decisions
- Leadership & Communication
- Use data to manage to performance and guide training initiatives
- Develop skill and inspire growth mindset in others through coaching and feedback
- Strong communication skills
- Exemplify ACS Values through CARE Actions
- Provide feedback to higher levels of the organization to inform quality and training initiatives
- Bilingual
CACP Program Expertise (Internal candidates):
- Strong working knowledge of Vida and other technologies to support day to day functioning
- Collaborate with other departments for the benefit of the patient
Education and Experience:
- Bachelor’s Degree (master’s preferred)
- 2 to 4 years of experience with people management experience
- Internal candidates: Successful tenure as a Facilitation or FOCUS employee for 6+ months post-onboarding
Licenses and Certifications:
- Current and unrestricted RN or LCSW, other licensed medical professions will be considered given strong ACP experience
Physical Requirements:
- Prolonged periods of sitting at a desk and working on a computer
